Liability Waiver

I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to StillyArts LLC, it’ officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “StillyArts”.

I hereby agree to release StillyArts and hold StillyArts harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.

I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. 

I understand that the facility is under video surveillance. Any picture or video taken at StillyArts  is the sole property of StillyArts and can be used for any purpose as deemed fit by StillyArts.

Age

I confirm that I am over 18 years of age.

Individuals under 18 years of Age/Children

Parent Consent and Liability Waiver

 Parent/guardian give consent for their child to participate in the Zumba,Art or Dance programs at StillyArts. I recognize the risks of injury common to any fitness program. I hereby waive and release StillyArts, volunteers and employees from any and against all claims of liability for any accidental injury or illness which may incur as a result of participating in the said fitness program including accidents or injury while on or about their premises, or while on off-site performances or activities. I hereby assume all risks connected therewith and consent my child to participate in said program. In the event of any emergency, I authorize medical attention from any licensed hospital, physician and/or medical personnel any treatment deemed necessary for my child’s immediate care and agree that I will be responsible for payment of any and all medical services rendered. Therefore, each person registering their family member should review their own health insurance policy for coverage. It must be noted that the absence of health insurance coverage does not make the employees of StillyArts to be automatically responsible for payment of medical expenses. This acknowledgement of Risk and Waiver of Liability has been read, understood completely and signed voluntarily by the parent/legal guardian of named student.

Photos and Videos

I understand that the studio is under constant video surveillance. I also understand that pictures and videos may be taken by authorized personnel.

I hereby grant the StillyArts LLC permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I hereby irrevocably authorize StillyArts LLC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

Code of Conduct

-No form of harassment is acceptable

-Participants and attendees will not be drunk or intoxicated

-Any damage to the studio, equipment, furniture etc. will be paid for my the participant/attendee

-Be respectful of fello dancers/attendees

-Follow Good etiquette and common sense

I agree to this voluntarily and of my own free will in exchange for the privilege of participation.

StillyArts reserves the right to expel anyone for any reason

Consent to Receive Electronic Communications

I agree to receive electronic communications from including but not limited to emails, electronic newsletters, text messages and Facebook messages.

Destruction of Premises and Equipment

Any damage to the studio, equipment, furniture or any property within the premises will be paid for by the participant/attendee. If the studio space has been rented out, then the sole responsibility of the damages lies on the renter. If the damage is greater than the deposit, the renter agrees to pay for damages within seven days of damages occuring.
